Mary D. White, MSN, ARNP, FNP-BC
Mary graduated from Seattle University in 2000 with a Master's Degree as an Advanced Registered Nurse Practitioner specializing in Family Primary Care. Prior to opening A Primary Care Clinic for Adults in 2005 Mary worked for a small practice in Kirkland, WA as well as Per Diem for Planned Parenthood.
Prior to receiving her Master's she spent one year in Long Term Care, one year in Medical/Surgical, 2 years at an Inpatient Psychiatric Unit and 12 years in Labor & Delivery as an RN.
Mary greatly enjoys all aspects of her patients' health care and feels strongly that everyone should be well educated about their care in order to make informed decisions. She especially enjoys seeing the same patients year after year, being able to get to know them and understand their healthcare & life goals.
Mary has lived in the Seattle area her entire life & has raised 2 beautiful children here.
Calista Ku, MSN, ARNP, FNP-C
Calista holds a Master of Science in Nursing - Advanced Community Public Health degree, a Post-Master's degree in Family Primary Care and is currently working towards a Doctor of Nursing Practice (DNP) degree at Seattle University.
Prior to joining A Primary Care Clinic for Adults Calista provided occupational health care as an ARNP. She also worked as a primary care RN at a community health center and has worked as a medical laboratory scientist in a clinical Oncology lab. Together with her experiences Calista incorporates a holistic approach to health care. Her philosophy encompasses both physical and mental health for optimal wellness. Calsita also focuses on preventative care, empowering individuals and communities to make informed decisions regarding health and wellbeing.
Calista grew up in the Pacific Northwest. She enjoys cooking, volunteering and hugging dogs.
Jaci originally started at A Primary Care Clinic for Adults in 2005, when it was first opened, as the receptionist. In 2008 she left us to pursue other opportunities. In 2010 she rejoined us after completing her training as a Medical Assistant.
Jaci has always lived in the Seattle Metro area & is currently living in Everett with her teenage son and their 2 dogs. She shares the philosophy that every patient deserves excellent health care & she strives to meet this every day.

Charges for office visits are based on the complexity of your condition, the time spent with the provider and the number of conditions/concerns addressed; your provider makes the determination as to the level of service. If you have any questions about our fees and payment options please contact the office.
As a courtsey to our patients we do bill insurance directly. However you must present a valid insurance card at the time of service. All co-pay's are due at time of service. If you are uninsured full payment is due when services are rendered. We accept Cash, Check, Visa & Mastercard.
Should A Primary Care Clinic for Adults need to utilize a collection agency for collection of payment on your account your care with the practice shall be terminated.
