Seasonal Vaccine Survey

COVID-19 vaccinations continue to decline. We would like to provide this vaccine to any patient that would like one. We need to know if you or anyone in your household would like to receive a COVID-19 vaccine this year.

There is a minimum order requirement for the COVID-19 vaccines. To avoid wasting vaccines, we would like to get a rough estimate of how many of our patients would like one, so we know how many to order.

As always, we do not require any patient to receive ANY vaccine.

Routine Vaccines

  • Hepatitis B
  • MMR
  • Pneumococcal
  • Shingles (Zoster)
  • Tetanus (Tdap)

Seasonal Vaccines

  • Influenza
  • COVID-19

Additional vaccines may be available upon request. If you need a vaccine not listed, please contact the office.
