This is not a complete list, please contact us if your insurance company is not listed. We also accept MVA claims, but only if you have PIP coverage, we will not bill third party claims.

Financial Information

Although we are contracted and considered “In Network” with most insurance companies, it is ultimately your responsibility to make sure that our provider is in your particular plan &/or network. It is also your responsibility to know your insurance benefits.

Charges for office visits are based on the complexity of your condition, the time spent with the provider and the number of conditions/concerns addressed; your provider makes the determination as to the level of service. If you have any questions about our fees and payment options please contact the office.

As a courtesy to our patients we do bill insurance directly. However you must present a valid insurance card at the time of service. We will bill your insurance, however if there is an issue later on you may be responsible for the bill i.e. you have dual coverage that you neglected to mention and the insurance requests their money back. All co-pay’s are due at time of service. If you are uninsured, full payment is due when services are rendered. We accept Cash, Check, Visa & Mastercard.

Should A Primary Care Clinic for Adults need to utilize a collection agency for collection of payment on your account your care with the practice shall be terminated.
